Patient and Family
Family is the most important part of this team because the cooperation and the serious
attempt to achieve the goals are the most important elements of a successful rehabilitation
program. The rehabilitation team (the multi-disciplinary team, the patient and his
family) will put the therapeutic goals that include the interests, desires, needs
and queries of the patient. The participation of both the patient and his family
is important in all stages of rehabilitation to achieve the required goals.
The Rehabilitation Team
The rehabilitation team evaluates the patient in all aspects and prepares a full
treatment plan that is individual and comprehensive of all patient needs. The plan
is executed after the approval of the family. After starting the intensive therapeutic
program, the patient’s condition is regularly evaluated. A home program can be given
to the patient upon his discharge according to the medical decision.
Consultant physiatrist examines and assesses the case closely, he conducts physical,
radiological and laboratory tests then provides the patient with medical rehabilitation
comprehensive program; he also collaborates with other rehabilitation team members
to make sure of providing qualified rehabilitation services through putting specified
goals with time stages to ensure that the patient achieved the desired goals of
treatment and rehabilitation.
The rehabilitation nurse and assistant nurse help the patient to apply the skills
that he have learned and encourage him to be independent and take care of his condition,
they also assist in patient/family education in how to take care of the patient's
condition after being discharged.
The case manager follows up the case of the patient and coordinates between the
patient and all the other parties such as funding entities, therapeutic team, and
some community institutes. He also answers all questions related to the effectiveness
and importance of rehabilitation. The Case Manager also facilitates and follows
on the process of patient discharge; he coordinates to find alternative care programs
or a continuation of rehabilitation in the outpatient clinics or under home health
care when the patient needs a continuous nursing or rehabilitation care after his
discharge or a treatment follow up at other medical bodies.
Patient Relations Coordinator
Patient Relations Coordinator assists the patient and his family in case of any
queries, suggestions, or concerns during the admission period in The City. And also
acts as a median in dealing with problems and solving them, in addition to meeting
the patient’s needs and providing him and his family with information related to
their rights and responsibilities in The City.
The Physiotherapist is specialized in the treatment of motor deficiencies
and the development of patient’s motor skills like walking, sitting and standing.
This is done in designated gyms and on sports equipment through individual or group
exercises until the patient becomes confident of his motor skills. He also teaches
the patient transfer methods and wheel chair skills. He also assesses the corrective
devices and recommends the type of required equipment such as crutches, wheel chair,
Occupational therapist teaches the patient self-care skills and activities of daily
life skills such as toileting, dressing, eating, and preparing the food as well
as the way to handle and manipulate objects taking into consideration the visual
and cognitive abilities of the patient. He also helps the patient in developing
the fine movements of hands and fingers in addition to giving the patient necessary
guidance before discharge.
Speech and Language Pathologist
Speaking and communication is essential for a person's healthy wellbeing. Therefore,
our speech and language pathologists work on offering therapeutic and training skills
to rehabilitate the patient to use his communication and hearing abilities better,
in addition to avoiding swallowing and breathing difficulties.
The health educator provides the patient and his family with all the health information
related to his condition in terms of the type of disease or injury. This information
can boost the overall health status and increase his self-confidence and participation
in therapy. Those services are provided to the patient and family through individual
counseling, group lectures, support groups and others.
The psychologist assesses the mental, emotional and psychological status. He deals
with any behavioral deficit resulting from the disease or injury. He discusses the
case with the rest of the team to reach a solution and a treatment that suits the
patient’s condition. Those plans to treat the behavior as well as other methods
of psychological therapy are designed to reach the maximum improvement that will
help to ease the patient’s integration into the society.
Diet has an important role in improving the patient's condition. The dietitian plans
a suitable diet for the patient and monitors nutritional health status. He also
answers all nutrition related queries.
The vocational rehabilitation therapist helps in assessing the skills, academic
abilities, characteristic features and physical abilities. He will help the patient
in sitting his vocational goals and the way to achieve them; when the patient is
ready to go back to his job or school, he will help him to do so.
The respiratory therapist helps patients with respiratory problems or patients with
the tendency to get respiratory problems. The therapist strengthens the muscles
that help the patient breathe properly avoiding congestion and respiratory problems
in the future.
The social worker provides advice, support and assistance to patients and their
families who suffer from social or financial difficulties during their stay in the
hospital. * All team members are keen to provide therapeutic rehabilitation services
in coordination with the group, the patient, and the family. Therefore, family conference
is considered an integral part of the team.
This conference is held according to the patient's need or request during his admission.
It consists of patient's family members and the medical rehabilitation team members.
In this meeting, the team answers all the patient's and discusses the plan of care,
the improvement in his condition and the expected time of his discharge. Patient
Education Sultan Bin Abdulaziz Humanitarian City provides information and educational
programs for the patient to enhance his awareness of his condition and health status,
and to clarify other aspects to response to the situations he might face or need
in his daily living. The education is also extended to the parents and care giver
in order to communicate with the patient in his home environment. This is done through
providing educational classes and meetings between the family and the therapists,
social workers, psychologists and the medical team in The City.
