Dear Patients, In order to provide the best level of integrated medical care for diabetic patients, Sultan bin Abdulaziz Humanitarian City (SBACH) has provided
a Diabetic Telecare Program (via website/phone) to
help those patients gain better
control of diabetes and reducing the risk of poor glycemic.
Vision:
The vision of this program derived from the vision of Sultan Bin Abdulaziz Humanitarian
City which is:
"Helping people to help themselves."
The Program’s Aims:
Due to the long distance, saving time and guarantee of the patient’s continuous
follow up, our City will follow up diabetic patients weekly through (website/phone)
for the importance of the following:

For early detection of
any poor glycemic control

Assistance in continuous glycemic
control to reduce the risk of poor glycemic control

To adjust glycemic medication
by a medical team through the phone as needed without having to cross long distances.

To provide help, advice and
guidance when needed by our program specialists team.
How to get follow-up by the Specialized Team?
Through our website, the patient can open a follow-up record within our Tele-care
program. Diabetic patient shall register on this page and for him/her to keep their
registration information therefore will be able to follow his/her own record and
to keep a printed hard copy of it.
After the registration the patient will be notified via email of his successful
registration. Then the patient will be asked to record his daily glycemic level
on his record page, which will be supervised by our specialized team providing him/her
guidance, and if needed the patient will also be contacted by phone.
